What a notice must include
Identify the copyrighted work, the allegedly infringing material and its URL, your contact information, a good-faith statement, a statement under penalty of perjury that the information is accurate and you are authorized to act, and your physical or electronic signature.
Counter-notice
If material was removed by mistake, send a counter-notice to the same address with your contact information, identification of the removed material, a statement under penalty of perjury that you believe removal was a mistake, and consent to the jurisdiction of the appropriate federal court.
Repeat infringers
Mackzy may suspend or terminate accounts that repeatedly infringe copyright or repeatedly submit abusive notices.
